Pre-Qualification Requirements for Construction Subcontractors 

Qualifications Information

 

To qualify for construction and construction-related opportunities, contractors must provide the following information. LLNS won’t initially ask for financial documents, but the contractor may be asked to provide financial documents in the future.

California Contractor License

A valid California Contractors license is required.


Surety

Name of present surety

  1. Surety must be listed on the U.S. Department of the Treasury's List of Approved Sureties.

 

Certification from the surety or its agent regarding:

  1. The number of times it has completed contractor's work in the past five years.
  2. Total bonding capacity of the contractor in dollars (Single and Aggregate).
  3. Available bonding capacity of the contractor at time of response in dollars.

*Professional Liability insurance required if Contractor is providing Architectural or Engineering Services as part of a Construction Subcontract (Design/Build)

**Contractors Pollution Liability is applicable if any work or activities involve asbestos, lead, or hazardous waste

 

Certification from the surety or its agent regarding:

  • Name of insurance carrier and local broker.
  • A certificate of insurance showing that the Contractor can meet the insurance requirements listed below. 

Safety Records

  • Completed “Subcontractor Safety Prequalification Questionnaire”
  • Provide a copy of the contractor’s corporate safety plan OR IIPP (Illness/Injury Prevention Plan). Please note that both may be required depending on the circumstance.
  • Provide a signed statement from the contractor's workers compensation carrier listing the contractor's California Experience Modification Rates (EMR) for the last three years. If your EMR rates are above 1.0 in the last three years, please provide a written explanation of the incident(s) and of the safety measures implemented to reduce the rate.
  • Provide a written explanation of the corrective measures that are being implemented by your company to improve its safety if any of the following occurs:
     
    1. Occurrence of any fatalities in the last three years,
    2. Receipt of any OSHA citations within the past five years,
    3. Days Away or OSHA Recordable Incident Rates for the past three years are above the national average found on Table 1 of the Bureau of Labor Statistics website (http://www.bls.gov/iif/oshsum.htm).
  • Provide the contractor's NAICS code for OSHA reporting purposes.
  • OSHA 300A Logs for years 2017 - 2023 (7 years)
     
    1. Include form 300 if there are accidents/injuries reported on 300A forms.
    2. Mitigation plan to prevent future occurrences.

 

Experience

  • Description of construction projects completed in the past three years including those federally funded (if any).
  • Value of the work performed by the contractor's own forces on all projects listed above.
  • Name, phone number, and email address of the owner’s representative for each project listed above.

 

Staffing

  • Provide a statement identifying the number of US Citizens employed, or the ability to staff projects adequately with US Citizens.
  • Provide evidence of enrollment as a Federal Contractor in the E-Verify System by providing a copy of the Contractor's ‘Company Information’ page printed directly from the E-Verify System.
     
    1. The copy of the ‘Company Information’ page from E-Verify System must show employer category information.
    2. Do not provide the form you used to enroll in E-Verify; do provide proof that your company has successfully enrolled as described above.
